Expert Senior Living Guidance - At No Cost to Your Family

Navigating senior housing and care transitions shouldn't feel overwhelming.  Founded by a RN with 12 years of clinical experience and leadership, Safe Haven Transitions  helps families across Washington evaluate assisted living, memory care, and adult family homes with complete confidence.  I handle the research, touring, and level-of-care matching so you can focus on what matters most - your loved one.

I help families understand the care their loved one actually needs, identify appropriate senior living options, and navigate the transition with an experienced RN at their side.

The Heart Behind the Care

Meet Eva Wagoner, RN | Founder & Lead Senior Living Advisor

My journey in healthcare covers more than a decade of hands-on experience. As a Registered Nurse for 12 years, I have worked in acute care, critical care, and hospice before transitioning into leadership as a Director of Nursing Services and later overseeing regional clinical compliance for various senior living facilities, hospices, and skilled nursing.

I’ve seen firsthand how confusing, overwhelming, and emotionally taxing senior living transitions can be for families. Too often, families are forced to make life-altering care decisions during a crisis; relying on either an overwhelmed case management system within the healthcare industry or automated online directories or sales pitches without knowing what truly happens behind closed doors.

I founded Safe Haven Transitions to change that.

As a veteran clinician, I bring an insider’s eye to senior placement. I don't just look at floor plans and amenities; I evaluate care capabilities, staffing standards, safety protocols, and regulatory compliance. My goal is to bridge the gap between complex medical requirements and your family’s peace of mind—helping you find a safe, high-quality, and dignified home for your loved one.

I also believe firmly that as humans we flourish where we feel most at home; so I combine clinical and safety insight with what is important to clients such as location and community preferences so that your loved ones feel like they are somewhere they truly belong.

I hold advanced nationally recognized certifications—DNS-CT (Director of Nursing Services – Certified), RAC-CTA (Resident Assessment Coordinator – Certified Advanced), and QCP (QAPI Certified Professional)—which reflect my deep expertise in clinical leadership, quality of care, and resident assessment. These mean when I walk into a facility, I am able to quickly see past marketing and identify the true standards of care happening behind the scenes.

When you work with me, you aren't just getting a tour guide, you're getting an experienced clinical insider who evaluates every community through the lens of a seasoned Director of Nursing.
